What it is
In plain language
Big changes bring a lot of decisions at once. Assist-Life Stage, Transition gives you someone to work through those decisions with, at a pace that suits you.
The support is practical. We help you understand the options in front of you, weigh them up, plan the steps and then follow through, including all the paperwork and phone calls that come with a move or a new role.
We coordinate with the other people in your life, such as family, school staff, employers, allied health and your support coordinator, so everyone is working to the same plan.
What this can include

Support is always tailored, so your plan may include some of these, all of them, or something different again.
- Planning the move from school to further study, work or day activities
- Exploring living options, including shared homes and living on your own
- Support to look at and understand housing, funding and service choices
- Help preparing for job seeking, interviews and starting work
- Setting up new routines after a move or a change in health
- Connecting with mainstream services such as housing, health and community supports
- Support to build the daily skills a new stage of life will need
